The questionnaire is used when you want to get information about your family from another person. It is easy for another family member to simply fill in the blanks with what they know. You can customize the questionnaire to include specific information you want and how many children to prompt for.
To print a Questionnaire:
1. Choose Questionnaire from the Reports tab on the Ribbon bar.
2. Set the options, as described below.
3. Click either Print or Preview to view the report.
The title is printed at the top of the first page and can look something like: "Jones Family Questionnaire."
Include Opening Paragraph
You can have the questionnaire printed with or without the opening paragraph text.
Include Christening/Baptism Line
You can have the questionnaire printed with or without a line for christening/baptism information.
You can have the questionnaire printed with any number of child information sections.
To include prompts for LDS ordinance information, select this option. Lines will be included for baptism, endowment and sealing dates and places.
You can specify how many blank lines you want to include for Other Information. Family members often know additional information that you will want.
Include Closing Paragraph
You can have the questionnaire printed with or without the closing paragraph text.
When formatting the page layout, you can have a horizontal line printed at the top and bottom of the page or a line all the way around, thus boxing in the information. You can also turn off the line printing if you want a cleaner look.
Click the Page Setup button to change page margins, print orientation, line spacing, paper size, border lines, heading and page numbering options, and footer options. See Page Setup for more information.
You can specify the font name, size and style for the title, text and labels used in the questionnaire.
The text for the opening paragraph is printed at the top of the first page and serves as an introduction to the questionnaire. You can include the purpose for which you are seeking information and a return address where the form is to be returned.
The closing paragraph is printed at the end of the questionnaire and can be a summary of the information requested, a plea for the return of the form and a "thank you" for their time.
Where to Send the Report
The report can be output to various formats. Make your choice by clicking on the appropriate option button in the lower right. The choices are:
Screen / Printer-Sends the report to either the Print Preview window or directly to your printer depending on whether you click the Preview or Print button.
Text File-Sends the report to a text file on either your hard drive or a floppy disk. You will be prompted for a drive and folder.
Windows Clipboard Creates the report in a text file format and then places the text on the Windows Clipboard. You can then paste the report into other documents, emails, comments, etc.
PDF File - Generates the report, displays it in the Print Preview window and then creates an Adobe PDF (Portable Document File) file. This file can be viewed with the free Adobe Acrobat Reader and easily sent to other people by e-mail. (This feature is part of the Deluxe Edition only.)
When you have set all the options, click either the Preview button to see an on-screen view of the report before it is actually printed, or click Print to have the report sent directly to the printer.
Click this button to restore default settings to the report.
Note: The report settings for the Questionnaire report are saved in a file called Question.usr in the [My Documents]\Legacy Family Tree\_AppData\Usr\ folder.
